Letter to confirm the DWP has given you more time to send your PIP claim form

Your full name

Put your full name as it appears on your PIP application.

Your address

Put your street, town and postcode on different lines.

National Insurance number

For example, ‘QQ 12 34 56 C’. Check your National Insurance card, benefit letter, payslip or P60.

Reference number

This is the reference number the DWP gave you when you called to start your PIP claim. It’s also on your PIP2 claim form.

When did you call the DWP to ask for a new deadline?

For example ‘14 January 2022’.

Explain why you asked for a new deadline

For example, ‘I’m going to be in hospital for a while’, or ‘I need more time because I need help from my local Citizens Advice to complete the form’.

When was the first deadline on your PIP2 claim form?

Check the deadline at the top of your PIP2 claim form.

When is the new deadline you agreed with the DWP?

For example ‘4 December 2022’.
